I play in a band as a drummer / guitarist, and i work on oil rigs (very loud places if you didnt know), so basically my hearing is shot.. as a band member you spend a lot of time with the same small group of people and you learn what each band member is trying to say by the way the look / mouth it..
aaand! when im playing guitar, in your mind you know what the song you are playing sounds like so you blank it out and communicate much more effecitvly because your playing goes into autopilot..
Back to the question, i suggest quiet places to practice the most effective sarging, then like Crowd writes, it gets easier the more you go (to the louder places) - and if you know how the targets will react (as you have rehersed it with others) you know what to look out for!
Also! another cheeky tip - when you are in a sweaty smelly nightclub, make sure you have some of your aftershave / cologne applied into your ears!! (just around the cartlidge bit) remember, she'll be leaning in to talk to you; so she'll be greeted with a plesent aroma of seduction - and chances are any good smell that was on your neck has been sweated off! - but ears don't sweat
