1.) Lifestyle/Money: I live in a small town and want to move by myself to a bigger city nearby. And I'm quite afraid to do it. I also don't have any history in work (for a good personal reason).
I'm super nervous about not making any money. I don't know what to do in order to make money. Not be a millionaire yet, just start out and be doing well.
So this is the first thing you need to take under control. Living in scarcity won't help you with your
confidence, and I can't think of anything larger that will mess up your confidence.
If you don't have any skills, you have labour skills. Get to construction and get paid by the hour.
It doesn't matter how much you make, you need to get yourself out of the MINDSET that
there is no money out there.
There is something to remember about work and money:
As long as you GET UP every day, and you put in the work, you will be OK.
But the thing is to get up and work.
In work you will find pleasure and freedom, so don't put this off one more day.
Offer help ANYWHERE. Clean the toilets if you have to. Mop the floors. When your
head hits the pillow at night, and you think of your day, know that you've put in your
best today, and you've made your bread and butter for the day.

2) My self-esteem: my inner game is kind of a mess. I see my self as having no worth. I'm very depressed and I am on meds for it. But I think lowest of myself and think I'm unattractive. And I'm short so people are always making fun of me and try to push me around.
Following the guidelines I gave you above, go to your nearest library and borrow the book
called
"6 Pillars of Self-esteem" by Nathaniel Branden.
He is the father of the self-esteem movement, so get into his work.
Your self-esteem and a sense of self-worth is CRUCIAL for anything you want to do in life. Don't allow this
one part determine how far you'll go in your life.
In his books he has exercises you can follow on a daily basis, like the sentence completion
exercises.
With your daily work and reading, you'll be on your good way to making a difference in your life.

3). Social life: my social life needs work, I'm not gaming anymore. I do meet up with friends on occasion but I don't have people I'd call my real friends. I DON'T have real love or acceptance in my life. And I'm kinda a mr. nice guy - won't step on toes or will offend people. I also am quite passive and realize I need girls in my life.
Your passiveness is a MIRROR of your life. You're not just passive with girls or social life, you are
passive all around.
You need to grip yourself and realise that
your passiveness is literally KILLING your life.
Your
passiveness is your #1 enemy you need to handle.
When you start getting yourself under control, everything else will fall in place man.
Hope i was able to make a difference in what you'll do next in your life.
Good luck,
