What could I have done better?

Ignored the fact she didn't understand the ice breaker. Just continue the play. It's already done it's job even if she's too fuck witted to understand the joke.

I think she's legitimately a moron... or not a strong english speaker...

That's also a gimmicky opener. I'm surprised you get responses with it - unless you're really good looking.

It's a shit opener AND she's a genuine nitwit. If you have any other options you would want to kill rather than fuck her after 20 mins.

Why were you trying so hard to milk the opener? Just say "Nevermind. My 6-years-old cousin told me that joke and thought it was funny." or just ask her something else. Girls don't follow the logic of the conversation - if you make her feel good or interested she won't care what you said earlier. Opening doesn't have to be something spectacular. If she doesn't get the joke, just move on - no big deal.
