You should have escalated at the end of the first lesson. Now that you're on the third she may think that you're not confident in your feelings for her. I'd say escalate during your lessons and make playful flirtatious jokes. Build rapport during the lesson and be sure to make it sexual (eventually) from there get the romantic date.
