easy, just be yourself, though i had the same problem when i was like 11-14, what i did was just started slowly becoming what i considered myself, as in if you were orginally gonna say something but held it in because your nervous or worried what a person might say to you or actions they might do, i would do it anyway(better being BOLD, than weak/shy ) , examples: i told a person who got on my nerves they were fat and should consider losing weight, 2: have a smart comment, share your 2 cents =D its worth every penny, 3: consider yourself a god umongst others, your the shit and everyone else is not (just don't go overboard on that one ) umm i can't think of much more but just remember be yourself in front of others, you may feel hesitant at first but believe me just fully commit to being your entire self will really help ya on this prob

Whenever I fell an urge to do something approval seeking or when I have doubts whether to do something that might annoy someone I rationalize the whole situation.

Other peoples opinion simply doesn't matter. They don't know you as you know yourself and if they don't like you then it is their loss.
