This one, at least for me, I consider it hard. It depends on both of you. On how she is, and behaves, and about your mastery at PUA. I'm no master, nor a PUA, but here's what I would do.

1. You need to start the connection slowly. If you start the connection she'll smell your intentions. If she bites the trap then it's on. Because no one talks to their exes back just to see how their doing.
2. A call, a text message, or an email are good options. But the best, I think, would be bumping onto her somewhere. Nothing is better than that. Meeting again triggers emotions, and brings back memories.
3. Act like you don't care, remember this attitude. If it goes well, it's ok, and if it doesn't its okay too. You need to have confidence, and better, show her that confidence. Talk back to her in conversations. Show her you're a man.
