Start talking to every girl/woman you meet everytime/everywhere you have a chance: waitress, store clerks, girl that hand out promotions in malls, on the bus/train etc.
And try to joke with them but not clownish (jim carrey) type of jokes but those that put you in a good light (try David Deangelo's cocky funny).
With time you'll learn how to approach anybody, you'll get over approach anxiety and also will build a cocky personality.

Also for charisma try to build a good composure and body language (voice, tonality, smile, head up etc) ; you can practice that in front of a mirror.

As for getting over the approach anxiety, basically just as the last poster said. If you have to, set a goal to approach 100 women in 1 week, then go to the mall and approach every damn HB7+ you can find, and then do it again and again and again.
