I am trying to figure out if it's me or if this happens to everybody. I get "nice too meet you"ed too quickly. I dont know if i am giving off a repulsive vibe or if my breath smells or if I appear too eager. I think it's the last one. when I approach a girl, I treat her like she is special. should i stop doing that?

I will give this example.
last night i was at a wedding. this girl and her mother were standing. no loud music. in fact, no music at all. the girl had her cell phone in her hand. I made eye contact, she made eye contact. I approached her and got really close to her. very close, our feet were almost touching. the reason I got so close is I wanted to create an intimate experience (mistake #1? should I keep some distance? I noticed most guys keep a distance)

fortunately, the mother was smart enough to leave at that moment. I told the girl "nice cell phone"
she said "thanks"
I said "what's your name?"
she said "my name is Sharon" I said my name, we shook hands. I really thought she liked me based on her tone of voice.
I said "how do you know them?"
she said something about being friends with the groom.
then she said nice to meet you "I have to leave"
WTF?

I think I appear too eager/I act like she is an angel from heaven/I act like I am really excited to be talking to her. I think, I am not sure though.

how eager do I want to appear? how can I give off the vibe "you are nothing special"?

You're over analysing. Measuring the distance between your and her toes isnt going to pick up the chick. Being direct and showing your intention will.

Standing on her toes and then telling her she has a nice phone sounds a bit weird to me.
