Why are you scared of warm-ups?
You have approach anxiety. It is so bad that you're not even able to enter social situations. At this point, your anxiety sounds so bad that even if you did enter, I think you'd probably fuck it up and end up going home depressed. You're not even close to being in state.
Listen, you need to acknowledge and accept the fact that you're going to fail a lot. Its going to take countless failures before you are able to achieve consistent success.
Stop being afraid of failure and pre-accept that its going to happen. You've read a million books, but now its time to forget them for the time being and do the newbie mission. Go into the field and start greeting random strangers.
