I usually recommend dinner or perhaps walk around the mall, and then see an evening movie. You build rapport and attraction at dinner, then the movie is the perfect dark setting for romance and kino.
You don't look for ioi's at the movie. You make them. But this is touchy. I've taken girls out on first dates and gone straight to the movie. Depending on the girl and what you've established beforehand, she might be ready to make out, or she might be really uptight and nervous and stuck in that still-getting-to-know-you mode. That's why the pre-movie activity helps. Get her comfortable enough beforehand and she'll be primed for a movie makeout session.
And once you are in the movie you absolutely can not be a pussy. I can't stress this enough. You can't be afraid to just grab her hand and hold it. Once you do that, the ioi is that she starts rubbing your hand or squeezing it. If she just lets it sit there like a dead fish until you let go, then that's not a good sign. Also, turn your head and look at her so she sees you looking at her, then when she looks back just stare into her eyes with confidence and sexuality. Don't look at her creepily or avert your eyes. Then you move in for the kiss. A small one at first, then go back for a longer one.
