ADVICE NEEDED - Where did I go wrong?

I was out with friends a few nights ago and this very attractive blonde with tattoos (just my type) approached me and asked me if I knew what song was playing in the club at the time. After I told her she invited me to join her on the dancefloor. She was giving IOI's the whole time we were dancing together and even responded positively to whatever compliance tests that I tried. She said she was 26 and I am 20 which didn't seem like a problem for her. After about 1 or 2 songs had passed we were making out, she was grinding on me, getting very touchy and it seemed like she was having a great time. Then all of a sudden she walked away leaving me on the dancefloor alone. I wasn't sure what to do so I just left and rejoined my friends. I saw her 2 or 3 times after that throughout the night and I got the impression she was avoiding me.

Has anyone experienced anything like this in the past? and where did I go wrong?

Also I have seen her in that club before so I am likely to in the future, do i approach or stay away? any advice would be greatly appreciated.

also forgot to mention that I was talking to a friend who knows her and it turns out she lied about what her name was, does this matter?
