the chances of your gf getting pregnant differ according to the time of the month she is at, she is at her most fertile from 10 days after her period
starts until about 9 days after that, because that is when she ovulates (ie on the 14th day after her period started) and because sperm live for several days inside her. Am only mentioning this because if you are going to take precautions at any time, or consider taking any chances at allthis is the time to be most super careful!!!
that said its best always to use a condom, and if in doubt your gf could get the morning after pill

failure rates for any kind of contraception come from the number of pregnancies that result per 1000 people using that method. for condoms its mostly ones that split or come off inside, for the pill its usually where girls forget to take it on time, or have stomach upsets that cause them to be sick and not digest the pill properly. the form of contraception with the highest rate of success is the mirena coil or IUD that a girl has fitted in her womb, but it doesnt suit everyone for various reasons - not really suitable for girls who havent had a baby yet i think.
